Health Insurance

In Washington State, most health insurance plans cover acupuncture treatment.  Contact your insurance company and complete the insurance verification form before your first visit to clearly determine the details of your acupuncture benefits. We participate with many local insurance plans.  Please remember that insurance contracts are between you and your insurance company.  It is your responsibility to know the services covered and the amount due as a co-pay at our office.

We are contracted as a preferred provider with most insurance companies including, but not limited to:

  • –Aetna (Annie and Olivia)
  • –First Choice Health Network (Annie)
  • –Kaiser Access, PPO, Options and HMO plans (Annie and Olivia)
  • –Lifewise (Annie and Olivia)
  • –Premera Blue Cross (Annie and Olivia)
  • –United Health (Annie and Olivia)
  • –Regence (Annie and Olivia)
  •     PIP (Personal Injury Protection claims) (Annie and Olivia)

For insurance companies with whom we are not contracted, you pay at the time of service for each visit and we will provide you with a receipt to submit to your insurance company for reimbursement.

In some instances insurance does not cover the amount billed for services rendered.  This may be due to a deductible to be met or to a specific service that is not covered by your insurance.  In such cases payment of the amount due is your responsibility.  If you do not have insurance, payment is expected to be paid in full at the time of your visit.

Please also understand that insurance does not cover the cost of Chinese herbs or supplements. You will be responsible for the cost of Chinese herbs and/or supplements should you choose to take them.

Payment

We accept payment at time of service by cash, check, or credit card.

Cancellation/No-Show Policy

Our time, like your own, is valuable and we require a minimum of 48 hours notice to cancel or reschedule an appointment.  If this requirement is not met, you will be billed for the full amount of appointment.  The full amount will need to be paid in full prior to any further treatment being received by our office.
